    Metabolic-associated fatty liver disease (MAFLD) is the most common chronic liver disease in the world and also the number one liver disease in China.Its incidence rate shows an upward trend year by year.If the degree of liver steatosis is aggravated,liver fibrosis may occur and eventually lead to liver cirrhosis and even hepatocarcinoma.All these are serious hazards to people's health.In recent years,many new changes have taken place in the diagnosis and treatment of MAFLD,which has become a research hotspot.This article reviews the global harm and health burden of fatty liver disease,the change of fatty liver naming and the diagnosis and treatment of metaboli-associated fatty liver disease.The purpose of this study is to provide reference for the early diagnosis,disease evaluation and new drug development of metabolic-associated fatty liver disease.

    The chemical composition of Dai Medicine Dichrocephala benthamii has been studied.The ethanol extract in Dichrocephala benthamii  was isolated and purified by silica gel chromatography repeatedly,and the structures were identified by spectral analysis and chemical methods.Results showed that fifteen compounds were isolated from the ethanol extraction of Dichrocephala benthamii and the structures were identified as piperine (1),capsaicin (2),dihydrocapsaicin(3),1H-indole-3-carboxylic acid,ethyl ester(4),1H-indole-3-carboxylic acid,methyl ester (5),3-formylindole (6),pomolic acid (7),floridiolide A (8),16-methoxycleroda-3,13-dien-15,16-olide-18-oic acid (9),16-ethoxycleroda-3,13-dien-15,16-olide-18-oic acid (10),friedelin (11),Kaempferol (12),quercetin (13),syringenin isovalerate (14),gallic acid (15).It can be concluded that compounds 1-12,14 were obtained from the genus of Dichrocephala for the first time.

    HPLC Determination of Ursolic Acid and Oleanolic Acid in Paulownia Leaves from Xiangxi

    An efficient,accurate and stable method for the determination of ursolic acid and oleanolic acid has been established.Then the contents of ursolic acid and oleanolic acid in Paulownia leaves in Xiangxi were determined.High performance liquid chromatography (HPLC) method was adopted,and the chromatographic conditions were as follows:(1) chromatographic column:TADE-PAK AF-C18 column(250 mm×4.6 mm,5 μm;(2) mobile phase:acetonitrile-methanol-0.5% ammonium acetate solution (Volume ratio 61∶18∶21);(3) flow rate:1 mL/min;(4) detection wavelength:210 nm;(5) column temperature:35 ℃;(6) sample size:10  μL.The results showed that the separation degree of oleanolic acid and ursolic acid was 1.73,which met the requirements.The linear relationship between ursolic acid and oleanolic acid was good in the range of 0.497 6~9.952 μg and 0.126 1~2.523 μg (r=0.999 9),respectively,The average recoveries of ursolic acid and oleanolic acid were 99.93% and 98.97%,respectively.The contents of ursolic acid and oleanolic acid in Paulownia leaves in Xiangxi were 4.54~7.07 mg/g and 2.50~3.20 mg/g,respectively.HPLC method has good separation effect,high sensitivity and high accuracy,and is a scientific and effective method for the determination of ursolic acid and oleanolic acid in Paulownia leaves.

    Correlation Analysis of Serum Selenium and Gestational Diabetes Mellitus

    Objective:To explore the correlation between serum selenium and gestational diabetes mellitus.Methods:From March 2020 to September 2021,360 pregnant women were selected as subjects according to the inclusion criteria,180 of whom were normal physical examination subjects with negative GDM diagnosis and were classified as control group,and 180  with positive GDM diagnosis received hospitalization and were classified as observation group.Methods:Selenium levels in different pregnancy periods of the same group and the selenium levels in same pregnancy period of the two groups were compared.Pregnancy outcomes and neonatal outcomes of the two groups were compared.Results:The serum selenium level of pregnant women in the third trimester was lower than that in the middle and early trimesters in the two groups (P<0.05),and the serum selenium level of pregnant women in the second trimester was lower than that in the early trimesters in the observation group (P<0.05).The incidence of adverse pregnancy outcomes and adverse neonatal outcomes in the observation group were higher than those in the control group.Conclusion:There is a strong correlation between serum selenium and gestational diabetes mellitus,and it is of positive significance for pregnant women to supplement selenium by detecting trace elements during pregnancy.

    Number of patients with Alcoholic Liver Disease (ALD) tends to increase year by year in China.It is important for clinicians to determine a treatment plan based on the condition and prognosis of each patient in a timely and accurate manner,and the Model for End-Stage Liver Disease (MELD) has been widely used to predict the prognosis of patients with various liver diseases.This paper reviews the study on the predictive value of MELD score for short-term prognosis of patients with ALD,so as to provide a reference for clinical treatment.

    Effect of LncRNA-ROR on the Hypoxia-Induced Gastric Cancer Metastasis and Invasion

    Objective To investigate the effect of LncRNA ROR on metastasis and invasion in the hypoxia-induced invasion and metastasis in GC.Methods Using quantitative real-time polymerase chain reaction (RT-PCR) to detect the expression of ROR ingastric cancer tissues and cell lines.Employing the over-expression technique were used to observe the effect of overexpression of ROR on human gastric cancer cell metastasis and invasion, the transfection efficiency was verified by RT-PCR in SGC-7901 and MGC-803.Results ROR was significantly down-regulated in gastric cancer tissues samples compared with adjacent normal gastric tissues, and also down-regulated inthe hypoxic—induced GC cells. Hypoxia significantly increased the migration and invasion potential of SGC-7901 and MGC-803,After over-expression of ROR the hypoxia-induced migration and invasion of SGC7901 and MGC-803 cells were dramatically diminished. Conclusion LncRNA-ROR expression is decreased in hypoxic GC and suppressed hypoxia-induced GC metastasis and invasion, which could be a new biomarkers, a potential therapeutic target of gastric cancer.

    Progress in the Diagnosis and Treatment of Primary Central Nervous System Diffuse Large B-Cell Lymphoma

    In order to further improve the understanding of primary central nervous system diffuse large B-cell lymphoma (PCNS-DLBCL), PubMed, CNKI and other databases were used to search related literature on the diagnosis and treatment of primary central nervous system lymphoma, and then The pathogenesis, clinical manifestations, diagnostic methods and treatment measures of PCNS-DLBCL are reviewed. PCNS-DLBCL is a subtype of extranodal non-Hodgkin’s lymphoma that can affect any part of the central nervous system. Its pathogenesis includes the presence of Epstein-Barr virus, increased expression of PD-L1 and PD-L2, MYD88 gene mutation and CD79B Gene mutations, etc.; clinical manifestations lack specificity, high degree of malignancy, easy to relapse, and poor prognosis; diagnostic methods include imaging examination, cerebrospinal fluid examination, immunohistochemistry and histopathological examination, pathological examination is the gold standard for diagnosis; PCNS-DLBCL treatment measures include radiotherapy, chemotherapy, autologous stem cell transplantation, and combined radiotherapy and chemotherapy. At present, induction chemotherapy based on high-dose methotrexate is the best treatment plan.

    Clinical Value of Combined Monitoring of Blood PCT, BNP and HBP Levels in the Prognostic Evaluation of Patients with Sepsis

    Objective To explore the clinical value of combined monitoring of blood PCT, BNP and HBP levels in the prognostic evaluation of patients with sepsis. Methods 100 patients with sepsis were selected as the subjects of this study, and the treatment period was from January 2020 to December 2020. All subjects were subjected to joint monitoring of changes in blood PCT, BNP and HBP levels, and the effect of the joint detection was analyzed.  In order to clarify the effect. Results After the treatment of the patients, 100 patients were divided into 60 patients in the treatment improvement group and 40 patients in the treatment failure group according to the treatment outcome. The blood PCT (4.01±1.27) Pg/mL, BNP (61.41±1.27) of the treatment group was improved. ) Pg/mL and HBP levels (29.11±1.16) Pg/mL were lower than the treatment-ineffective group (P<0.05). The positive rate, sensitivity and specificity of the combined detection of the three indicators were 98.00%, 96.00%, and 92.00%, which were significantly higher than the single monitoring and combined detection of the two indicators, and the difference was statistically significant (P<0.05). Conclusion  The combined monitoring of blood PCT, BNP and HBP levels can effectively evaluate the prognosis of patients with sepsis.

    Effects of Atorvastatin on the Epilepsy Model of LI-PILO Rats

    Objective To study the neuroprotective effect of atorvastatin (ATV) on lithium pilocarpine chloride (LI-PILO) epilepsy model in SD rats. Methods  Induced status epilepticus (SE) model by intraperitoneal injection of LI-PILO, Record the behavioral changes of SD rats in the ATV group and the control group; Simultaneously use ELISA reagents and Nissl staining to detect serum inflammatory factor levels and neuronal damage in the chronic phase of the ATV group and the control group. Results  In the ATV group, serum inflammatory factors decreased significantly in the chronic phase, the SE Latency and Latent period of epilepsy were significantly prolonged, the level of seizures in the acute phase decreased significantly, and the spontaneous seizures in the chronic phase reduced significantly. Nissl staining showed that more neurons survived in the hippocampus. Conclusion  Atorvastatin may play a neuroprotective role by inhibiting the expression of inflammatory factors and reduce seizures in LI-PILO induced epilepsy model.

    Obejctive To analyze and compare the effect of different courses of retroauricular injection of methylprednisolone on sudden sensorineural hearing loss. Methods 134 patients with sudden sensorineural hearing loss who were hospitalized from February 2019 to December 2019 were randomly divided into 3 groups according to the order of admission. 45 patients in group A were injected with methylprednidone 40 mg retroauricular, a total of 3 times. 45 patients in group B, a total of 5 times. 44 patients in group C, a total of 7 times. After thetreatment, pure-tone audiometry was re-examined to compare the efficacy of the three groups. Results the effective rate of group A was 64.4%, the average threshold of hearing increased 20.2±17.5 dB HL, the effective rate of group B was 66.7%, the average threshold of hearing increased 20.9±16.7 dB HL, the effective rate of group C was 68.1%, the average threshold of hearing increased 19.6±17.9 dB HL, there was no statistical significance. Conclusion there is no significant difference in the efficacy of retroauricular injection of methylprednil in the treatment of sudden sensorineural hearing loss, a short course of retroauricular injection of methylprednil can achieve the purpose of treatment.

    Objective  To investigate the effects of exercise breathing exercise rehabilitation therapy on lung function, exercise endurance and quality of life in patients with stable chronic obstructive pulmonary disease. Methods  80 patients with rstable chronic obstructive pulmonary disease were selected as the research objects, and they were randomly divided into an experimental group and a control group, with 40 cases in each group. The experimental group was added to the control group, and the respiratory exercise rehabilitation therapy intervention. The difference in lung function (FEV1, FVC, FEV1∶FVC) between the two groups of patients before treatment and one month after treatment was compared. The difference in quality of life (CAT score) and exercise endurance (6MWT distance) between the two groups at discharge and at 3 months after discharge was also compared. Results  After 1 month of treatment, the FEV1, FVC, FEV1∶FVC values of the experimental group were higher than those of the control group; both at the time of discharge or 3 months after discharge, the CAT values of the experimental group were lower than those of the control group, and the 6MWT values were higher than those of the control group.Conclusion Rehabilitation therapy intervention of breathing exercise exercise has a good effect on enhancing the lung function of patients with chronic obstructive pulmonary disease in stable stage, improving their exercise endurance, and improving the quality of life of patients.

    Effect of Propranolol Combined with Amiodarone on Malignant Arrhythmia

    To investigate the effects of propranolol combined with amiodarone on inflammatory factors and QT interval dispersion in patients with malignant arrhythmia. 92 patients with malignant arrhythmia treated in our hospital from February 2018 to June 2020 were randomly divided into two groups with 46 cases in each group. The control group was given amiodarone hydrochloride tablets on the basis of routine treatment, and the observation group was given propranolol hydrochloride tablets on the basis of the control group. The clinical efficacy, inflammatory factors, QT interval dispersion and adverse reactions were compared between the two groups. The results showed that the total effective rate in the observation group was 93.48%, which was higher than 78.26% in the control group (P<0.05). Tumor necrosis factor after treatment α, Interleukin-6, C-reactive protein and QT interval dispersion were lower than those before treatment, but more in the observation group (P<0.05). There were 3 cases of adverse reactions in the control group, which had no significant difference compared with 4 cases in the observation group (P>0.05). It shows that propranolol combined with amiodarone can significantly enhance the clinical efficacy, alleviate inflammatory reaction, reduce QT interval dispersion and have higher safety than amiodarone alone.

    Value of US-CNB Histology in the Diagnosis of Early Invasive Lobular Carcinoma of the Breast

    Objective To analyze the value of ultrasound-guided thick needle aspiration (US-CNB) histological examination in the diagnosis of early invasive lobular carcinoma (ILC) of the breast. Methods 169 patients with early breast cancer diagnosed in the breast surgery department of a third-class hospital in Wanbei, Anhui Province between October 2019 and October 2020 were selected as the research objects. Postoperative histopathological results (gold standard): early breast ILC 65 Cases, 104 other early breast cancer patients. All patients underwent ultrasound-guided fine-needle aspiration (US-FNAC) and US-CNB histological examinations before surgery, compared with postoperative pathological examination results, and recorded the value of US-FNAC and US-CNB in the diagnosis of early breast ILC, and all patients Comparison of the positive expression rates of estrogen receptor (ER), progesterone receptor (PR), and human epidermal growth factor receptor-2 (HER-2) tested by US-CNB. Results US-FNAC diagnosed 46 patients with early breast ILC and 123 other patients with early breast cancer. US-CNB diagnosed early breast ILC in 57 cases and 112 other early breast cancer patients. The sensitivity of US-CNB in diagnosing early breast ILC was 84.65% higher than that of US-FNAC 68.18% (P<0.05), but The accuracy and specificity of the two detection methods were not statistically significant (P>0.05); the positive expression rate of HER-2 in early breast ILC patients was higher than that in other early breast cancer patients (P<0.05), and the positive expression of ER and PR Compared with other early breast cancer patients, the rate was not statistically significant (P>0.05). Conclusion US-CNB histological examination has good sensitivity in diagnosing early breast ILC, and the positive expression rate of HER-2 in patients is high.

    Psycho-cardiology refers to cardiovascular disease with mental illness which is attracted growing attention with medical development. In the previous treatment of cardiovascular disease, doctors focused more on organic cardiopathy than on psychological disease of patients. Numerous studies have shown that intestinal flora and its metabolites played a vital role not only in cardiovascular disease, but also in mental and psychological diseases. This article summarizes the influences and mechanism of intestinal flora and its metabolites in the outbreak and development of psycho-cardiology, as well as the potential value in the prevention and treatment of psycho-cardiology to offer new insights into the prevention and treatment of ppsycho-cardiology.

    Study Objective:This article reviews the clinical manifestations and diagnosis and treatment of uterine scar pregnancy after cesarean section in recent years at home and abroad, with a view to providing a reference for gynecologists in the diagnosis and treatment of uterine scar pregnancy after cesarean section.Measurement: Search CNKI, Wanfang, Pubmed and other databases, and use literature tracing and other methods to collect publicly published relevant research on uterine scar pregnancy after cesarean section.Results: The main diagnostic methods are: ultrasound examination, MRI, hysteroscopy, laparoscopy, and histopathological examination are still the gold standard for the diagnosis of CSP. There are many ways to treat CSP. At present, level I evidence has not been proposed, so that there is no specific evidence-based guideline. The current treatment methods are: drug treatment, uterine artery embolization, surgical treatment or multiple treatments. Clinical treatment of CSP patients should clearly identify the type of disease and the advantages and disadvantages of various surgical procedures, combined with comprehensive analysis of the patient's condition, fertility requirements, economic conditions, hospital facilities and other indicators, and select a reasonable surgical method to minimize patient treatment. Risk, obtain a good prognosis and preserve fertility.Conclusion: For the early diagnosis of CSP method has been clear, but the best treatment plan and management guidelines at present has not yet reached a consensus that the uterus after cesarean section scar pregnancy diagnosis and treatment, there is no uniform standard, the most ideal treatment need large sample randomized controlled studies data.

    Influence of Lower Limb Movement on Muscle Strength in COPD Patients with Invasive Mechanical Ventilation

    To compare the influences of exercise mode on COPD patietns with   invasive mechanical ventilation, 112 COPD patients who stayed in ICU conducted a group controlled experiment. According to the predetermined proposals, two groups of patients took different lower limb movement. The intervention group performed in-bed bicycle exercise, while the control group performed traditional in-bed exercise. The results indicated that there were significant differences subjective and objective indicators of lower limb, level of activation in ICU when patients left ICU as well as Barthel index scores when patients left ICU and discharged from hospital respectively between two groups (P<0.05). There were no significant differences in upper limb muscle strength measured either by electronic grip gauge or MRC scale between two groups (P>0.05). There was no significant difference in Barthel index score measured in the first six months after discharge (P>0.05). The results show that early in-bed bicycle exercise is secure and workable for COPD patietns with invasive mechanical ventilation, which can improve lower limb muscle strength and mobility and  promote recovery of activities of daily living.

    Investigation on 8 genetic characteristics (including the retroflexion,twisting tongue,eyelids,forehead hair,hair vortex,earlobe,pollical type,little finger bending) has been conducted among 1 164 college students (705 Han people and 459 Zhuang people).The statistical analyses and the cluster analysis have been carried out between different nationalities in other regions.The results showed that,among Han and Zhuang population in Guangxi,there were significant differences in the retroflexion,forehead hair,and the little finger bending,but there were no significant differences in  pollical type,twisting tongue,eyelids,earlobe,hair vortex.Clustering analysis showed the shortest genetic distance between the Han population and Zhuang population in Guangxi,and the longest genetic distances between Han and Zhuang population in Guangxi and Mulao population in Guizhou (0.470 6,0.512 5).

    Special Staining Pathologic Changes and the Expression of NF-κB After Diffuse Axonal Injury in Rats

    To observe the pathologic changes and the expression of nuclear factor-kappa B(NF-κB)after diffuse axonal injury(DAI) in rats,its mechanism is firstly explored.The rat DAI model was established.Immunohistochemical SABC method was used to observe the expression of NF-κB,combining Glees-Marsland and Weil staining to observe the pathologic changes in different stage of DAI.Swollen and ruptured axon were found in brain tissue 1 h after DAI,and myelin sheath showed edema,delamination,marked axonal retraction ball and screen shape changes were detected after 24 h,and those morphological changes reached the peak after 72 h.The expression of NF-κB could be detected 1h after injury,the positive cells increased after 3 h,peaked after 24 h,and kept this tendency for 72 h.The positive cells decreased after 7 d.The control group showed negative staining.The findings showed that the expression of NF-κB increased in rat DAI model.The NF-κB may play an important role in secondary axonal injury.

    Community-Based Rehabilitation Demand of Stroke Patients in Xiangxi Autonomous Prefecture and Influential Factors

    To obtain the information of community-based rehabilitation demands of stroke patients in Xiangxi autonomous prefecture and the relevant influential factors,stratified cluster sampling method was used to conduct a questionnaire survey among 206 stroke patients in Xiangxi.Results showed that 46.6% patients had never heard of community-based rehabilitation; the community-based rehabilitation demands mainly included rehabilitation training,rehabilitation treatment,rehabilitation knowledge popularization,and rehabilitation nursing.Multiple factor logistic regression test showed that the number of patients' children was the first influential factor on rehabilitation knowledge popularization and rehabilitation demand,medical expense was the first influential factor on rehabilitation referral service demand,and age was the first influential factor on rehabilitation nursing demand.It can be concluded that stroke patients in Xiangxi have low degree of awareness of community-based rehabilitation,community rehabilitation demands are diversified,and the number of patients' children,medical expenses,and age have important impact on the community-based rehabilitation demands.

    Effectand Mechanism of Glutamate Release at Hyperxia Induced Retinal Injury in Newborn Mice

    The concentration change and mechanism of glutamate at hyperxia induced retinal injury in newborn mice retinal injury are explored.The experimental group was put  in 95% hyperxia  environment to induce retinal damage,and the control groups in normal air.HE staining was used to observe retinal injury,ELIASA  to detecte the glutamate (Glu) concentration,and reverse transcription polymerase chain reaction(RT-PCR) to analyze the expression of retinal glutamate transporter (GLAST) and glutamine synthetase (GS) mRNA.The results showed that,compared with control group,hyperxia treatment group with retinal injury showed the increased  Glucontent,and decreased GLAST and GS mRNA expression.It thus can be concluded that hyperxia can lead to the increase of retinal glutamate in newborn mice,the mechanism of which was associated with decreased GLAST and GS mRNA expression.Glutamate increase may be one of the reasons for the hyperxia induced retinal injury.

    The cervical cerclag in the treatment of  caused by uterine anomalies was in estigated.Forty cases were divided  into 2 groups.Twenty of them who received the treatment were of the experiment group,while the rest 19 cases who refused the treatment were of the control group.The respective outcomes in pregnancy were observed.The success rate in pregnancy of the experiment group was 90.47%,while that of the control group was 15.79%.Obviously,success rate of the former was much higher than that of the latter.The difference was statistically significant(P<0.05).It can be concluded that the  transvaginal cervical cerclage is a safe and effective method in the treatment of recurrent abortion caused by uterine anomalies.

    Objective: to study the influence of viola diffusa gine (VDG) extract on myocardial cells.Methods:myocardial cells were cultured with different concentrations of VDG extract to intervene.The myocardial cells pulse frequency and the contents of lactate dehydrgenase(LDH) in culture solution were determined.MTT method was used to observe the cells survival rate.Results:free radicals injured myocardial cell,VDG extract reduced LDH leakage and increased the cell survival rate (P<0.05).Conclusion:The results indicate that VDG extract has a certain protective effects on myocardial cells injured by free radicals.

    Survey on Treatment Compliance of Patients with Chronic Obstructive Pulmonary Disease

    Objectives:to explore the treatment compliance of patients with COPD and influential factors.Methods:162 patients with COPD were accepted in the sampling survey by questionnaire about the related knowledge and treatment compliance of COPD during stable phase.Results:40.7% of the patients were on better level of treatment compliance.The result indicated that the influential factors of patient's treatment compliance included the course of disease,regular re-examination,the awareness  level of the disease,the relationship between patients and doctors,and the economic burden.Conclusion:the compliance level of the patients with COPD is on low-middle level.The major influential factors are the awareness  level of the disease,the relationship between patients and doctors,the course of disease,economic burden and regular re-examination.Improving the doctor-patient relationship and strengthening health education can increase the treatment compliance.
